Larva/Tiamat is an alternative Tiamat from another world, coming from the same universe as Isabel. Larva/Tiamat's class name is Alter Ego. She's the Alter Ego Servant summoned by Isabel in her own Universe.

The Primordial Mother of Sumerian mythology that answered the call for a young Isabel for a mother figure. Larva/Tiamat, a projection of the actual Tiamat, came to her aid and raised Isabel like her daughter. Isabel was rejected by her own mother and, as such, the young girl was left to fend for herself. Her wish for a mother touched Beast II Tiamat who used her powers to create a mental projection who got contracted by Isable as the Alter Ego-class Servant Larva/Tiamat.

Of course, such an unusual summon didn't came unnoticed and an organization called Chaldea immediately got into action and apprehended the pair. Since neither of them showed any signs of aggression, they got both included in Chaldea as one of the Master and Servant.

It was Tiamat's care and education that gives Isabel her love for stepford wives. After all, Larva/Tiamat is the ultimate stepford wife of all. Soon, Isabel and Larva/Tiamat rose the ranks to become the strongest Master/Servant duo in Chaldea. She was sent by the organization to the universe hosting the "Main" Sanctuary, in hopes to make contact and have cordial relationships. This is why we see Isabel and a second Tiamat living in the Sanctuary.

Do not be deceived by Larva/Tiamat's gentle and motherly nature. Even in this form, she is still incredibly powerful and more than capable to reduce a country into ashes.

But, of course, that would only happen if Larva/Tiamat got royally, divinely pissed for some reason. For now, she haven't shown any signs of wanting to blow up the whole country, being too busy being loved by and loving Isabel.
